No Registration Packet?

1. Meals. - Deadline: postmarked by September 15

   Problem:

   Two lunches and dinner Friday night are included in your registration fee only if your registration is postmarked by September 15. You must tell us if you want a vegetarian meal by September 15th also.

   Reason:

   The deadline is set by the caterer so they can order the right amount of food to serve us.

   Suggestion:

   School districts can be slow with funding\processing convention registrations. It would be better to register yourself and have the school district reimburse you.

   If you register on-site or after September 17th, you will need to arrange for your own meals.

   
   

2. Room Reservations: - Reservation deadline: September 24, 1999

   Problem:

   CAHPERD does not reserve sleeping rooms for the convention.

   Reason:

   CAHPERD can not be responsible for paying for rooms that are not used.

   Suggestion:

   Make your own room reservations and find your own roommate(s) if you wish. Make your reservations early to guarantee a room. Call (303) 695-1700 or (800) 962-7672. Reservation deadline is September 24, 1999.

   
   

3. Receiving Credit for Recertification:

   Problem:

   CAHPERD offers credit for recertification as a service to its members. Adams State College and CDE set the requirements that CAHPERD must follow to offer this service.

   Registration for credit will be by Adams State College and CDE designated representatives:

   1. Thursday and Friday morning only at the registration desk
   2. Verification forms must be signed immediately following each session you attend by the presider or by any CAHPERD Executive Board member at that session. The Board members will be wearing red ribbons on their name tags.
   3. The verification forms must be turned into the registration desk at the end of Saturday's sessions.

   Reason:

   The University and CDE must follow strict regulations for giving credit to individuals. Convention credit is not intended to be "easy credit".

   Suggestion:

   Decide if you really want to follow these requirements. Register on-site on Thursday or Friday morning only if you want to follow all the requirements. More credit information.

   Do not include fees for credit with convention registration.

   
   

4. Student Reimbursement

   Students who volunteer to help with convention duties (minimum of 4 hours) will receive registration reimbursement. To be eligible, contact Dr. Joe Quatrochi at (303) 556-2898.

   
   

5. Hotel Accommodations

   You must make your own reservations directly with the Holiday Inn - Denver Southeast at (303) 695-1700 or (800) 962-7672. The Holiday Inn - Denver Southeast is a beautiful facility with a complimentary airport (DIA) shuttle, access to the International Athletic Club for $5 per day, walking distance to Cherry Creek State Park and a wonderful view of the mountains.

   Do not send any money to CAHPERD regarding your hotel stay.

   When you call, be sure to identify yourself as a CAHPERD convention participant to qualify for the discounted room rate of $72.00 (single or double occupancy) per night. Make your reservations early!

   
   

6. Check List
   • Pre-registration form filled out completely -Part I and Part II
   • Check marks next to meal functions you will attend
   • Have one check made out to CAHPERD for entire total of membership dues, pre-convention and\or convention registration fees.
   • Made a photocopy of the registration form for your records.
   • Previewed opportunities for student reimbursement
   • Previewed opportunities for Adams State College or CDE credit
   • Made your room reservations with the Holiday Inn Denver Southeast.

1. CONVENTION REGISTRATION
   • Two (2) Choices:
     • PRE-REGISTRATION: DEADLINE: postmarked by SEPTEMBER 15, 2000. Includes all meals for the day(s) you are attendig and all convention materials
     • On-Site Registration: On the day(s) of the convention. Includes all convention materials.
   • Complete the entire convention pre-registration form and/or CAHPERD membership form, one for each registrant.
   • If you are joining or renewing membership at this time, you may register at the member rate. If you have any questions about your membership, please call CAHPERD's Executive Director at (719) 274-4331.

   
   

2. MEALS
   • If you want to join us for meals, you must pre-register (before September 15, 2000) for the convention.
     On-site registration does not include meals.
   • Please check the appropriate boxes for meal selection. No checks mean no meals will be ordered for you.

   
   

3. PAYMENT
   • We do not accept purchase orders or school vouchers as payment. Please have your school district reimburse you.
   • Payment must accompany pre-registration. Please make one check for the entire total (Registration, shirts, and/or Membership). Make checks payable to CAHPERD.

4. CONFIRMATION
   • If you would like confirmation of your registration, please fill out your e-mail address on the registration form.

5. PRE-CONVENTION WORKSHOP REGISTRATION:
   Please check the appropriate box on the registration form for your 1 choice of the 3 workshops.
   Remember: you must stay with your first choice for the duration of the pre-convention workshop.
   Remember: we must have at lest 20 participants registered for the Physical Best workshop before September 15th or this workshop will be cancelled.

   
   

6. 2000CAHPERD CONVENTION POLO SHIRTS:
   Description: White cotton shirt with red embroidered 2000 CAHPERD logo, $15.00 per shirt. Please check the appropriate box on the registraton form for the polo shirt size and amount you need.
